Hoe zoekfunctie maken in formulier access 2003

Ik moet voor schoolopdracht een zoekfunctie maken in een formulier.

Zo ziet het formulier er ongeveer uit:

Wat ik dus wil, is dat als ik bijvoorbeelde bij naam Jan invul, dat ik alle gegevens van jan krijg. Maar ik moet ook bijvoorbeeld door het telefoonnummer alle gegevens kennen verkrijgen.

Heb al op internet gezocht maar kan het nergens vinden.

Iemand een idee hoe het moet?
